We want someone who brings more to the table than just skills "” a visionary whos ready to innovate and inspire.Mission-Driven Leader: Upholding Rooted Lifes mission with dedication and ethical leadership.Evidence-Based Practitioner: Utilizing approaches like Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.Community Connector: Deep connections and relationships in the San Diego homelessness provider communityActive Engager: Conducting outreach and engagement through phone calls, mail, and community visits.Client Advocate: Coordinating with healthcare professionals and advocating on behalf of clients.Professional and Adaptable: Maintaining professionalism and adaptability in a rapidly changing care environment.Efficient Organizer: Keeping client and care records meticulously organized and up-to-date.Multitasking Pro: Excelling in managing various tasks and prioritizing effectively to meet program and client needs.Team Advocate: Promoting open communication and cooperation among team members and clients.Team Leader: Supporting and managing the LCM team, ensuring high-quality service delivery and professional growth.What You'll Be Doing:Care Management Responsibilities:CalAIM and ECM Implementation: Extensive understanding of CalAIM initiatives and Enhanced Care Management (ECM) to deliver comprehensive, person-centered care. Ensure all care coordination activities align with ECM guidelines and requirements.Care Plan Oversight: Overseeing the development and implementation of Individual Care/Health Action Plans for each client.Flexible Service Delivery: Offering services in a variety of settings, including office-based, telehealth, or community-based services, based on client convenience.Outreach and Engagement: Conducting activities to facilitate linkage to the ECM program, including phone calls, mailed information, and community visits.Needs Assessment: Conducting individual assessments to understand client needs and disseminating information accordingly.Service Coordination: Coordinate services from social service providers to ensure ECM goals and CalAIM objectives are met.Medical Coordination: Schedule and track medical appointments, maintaining updated files that align with ECM documentation standards.Transport Logistics: Arranging transportation for clients as required.Legal and Probation Support: Assisting program participants with Probation and Parole requirements, including providing progress reports and supportive documentation.Client Advocacy: Coordinating and advocating on behalf of clients with healthcare professionals, with the clients permission.Community Connection: Connecting clients to essential medical services, social services and support groups as needed.Client Support: Accompanying clients to office visits as needed and appropriate.Progress Evaluation: Regularly evaluating client progress and updating goals as necessary.Mental Health Promotion: Actively promoting mental health and wellbeing among clients.Timely Documentation: Complete all documentation promptly in our EHR, ensuring compliance with ECM and CalAIM reporting standards.Team Collaboration: Attending weekly staff/team meetings and participating in supervision sessions.Continuous Learning: Engaging in assigned training sessions, such as Motivational Interviewing and ACEs Certification.Data Management: Collecting and entering client data as needed. Keeping EHR/EMRPositive Environment Cultivation: Maintaining a positive, life-giving environment for clients and keeping client files updated with all relevant medical, legal, and social service documents.Team Leadership Responsibilities:Team Supervision: Provide oversight and support to Lead Care Managers, ensuring alignment with Rooted Life's mission and quality standards.Performance Monitoring: Track team performance, address challenges, and provide constructive feedback to foster professional growth.Task Delegation: Organize and prioritize workloads to ensure client needs are met efficiently.Training and Support: Train and mentor team members on ECM processes, CalAIM guidelines, and best practices in care management.Collaboration Facilitation: Lead daily meetings to enhance team communication and problem-solving.ECM Compliance and Quality Assurance: Ensure the team consistently meets ECM and CalAIM requirements, maintaining high standards of care coordination and documentation.In this role, you will not only shape the care journey for our clients but also lead a team dedicated to making a difference.
