Full-Time Nurse Practitioner - Flushing - ClinicalReports To: Director of Medical Affairs137-43 45th AvenueFlushing, New York, 11355United States Responsibilities:Involves with direct patient physical assessment, education, and health planningMaintains continuity of care by regular patient follow-upAssists in the coordination of services with other disciplines and patient''s insurance plansAdvocates for patient in arrangement of medically necessary servicesCommunicates with community''s resources to ascertain adequate assistance for patients in needRefers patients to specialty services when necessaryConsults with collaborating physician and other members of the health care team to establish plan of careParticipates in quality assurance process to assure optimal care deliveryProvides comprehensive case management services that include emphasis on health promotion and disease preventionMonitors patient outcome in accordance to the practice protocolsCollaborates with staff nurses and nursing administrators with assessment and clinical decision in patient care planningActs a resource person for the nursing staffConducts education in service on practice issuesOther duties as assigned Minimum Qualifications: Graduated from approved Nurse Practitioner ProgramLicensed as Nurse Practitioner in New York StateBasic CPR (BCLS) certification requiredAt least 1 to 2 years of clinical experience as a primary care Nurse PractitionerBoard/Specialty Certification preferredAbility to speak Mandarin, Cantonese, Korean or Spanish is a plus
