Job DescriptionAssumes responsibility and accountability for the nursing assessment and/or primary care for a selected population of senior patients. Provides annual wellness assessments for senior patients in variety of settings. Maintains currency of licensure and certification. Functions under the collaborative practice agreement for delegated medical aspects of care with collaborating physician(s).Essential Duties and Responsibilities include the following:Provides anticipatory guidance, screening and health education to patients to promote and maintain health. Provides annual health assessments for senior patients in a variety of settings such as in our Senior Wellness Clinic, in various provider offices and at the patient''s home.Assesses senior members for preventative interventions such as safety, fall risk, depression screen, and urinary concerns.Evaluates the chronic conditions of senior structs individual and/or families or groups about health or defined health problems. Provides education to patient and family members based on screenings, assessments and the patient''s chronic conditions.Plans appropriate care based on screening and chronic care guidelines.Plans appropriate care utilizing community resources and acts as a liaison by relaying information necessary for continuity of care to other health professionals and agencies. Flexible with the setting of the evaluations based on company need.Promotes guest relations; anticipates and responds to the needs of others; provides assistance in a courteous, quick manner; treats others with care and respect maintaining privacy, confidentiality and dignity. Maintains a professional appearance and a clean, safe environment. Participates with the company''s Quality and Coding departments to ensure the requirements established by Centers for Medicare and Medicaid Services (CMS) are fulfilled for each senior patient.Provides clinical services by managing patients in the home or clinic.RequirementsEducation and/or Experience:Active California Nurse Practitioner LicenseActive California RN LicenseCurrent BLS certificationValid California Driver''s License and proof of insurance.Demonstrated proficiency in use of Microsoft Office, Word, Excel, and other applications as requiredExcellent observational, verbal, and written communication skillsInterpersonal skills necessary to work productively with patient, families, nursing staff, and other departmentsSpanish speaking helpfulThree to Five years experience as a Nurse Practitioner - Preferred
