Family Practice/Primary Care Nurse Practitioner

We are seeking a nurse practitioner (NP) or physician assistant (PA) for primary care rounding in a behavioral health facility in the Holyoke, MA area. This is a full-time role with no on-call requirements. Schedule: 4/5 Days per week8am - 4pmNo Call Some weekend and holiday coverage requiredResponsibilities:Perform primary care in a specialty setting Manage chronic disease, diagnosis illness and medical conditions, and implement plan of careOrder and interpret lab resultsPrescribe and monitor medicationsConsult with physicians and coordinate patient care with other healthcare practitionersEducate patients and their caregivers on coping skills Build relationships with your patients, patients family members, and facility staff to facilitate effective treatment deliveryApproximately 15 consultations per dayRequirements:Primary care experience requiredNew grads encouraged to applyActive and unrestricted MA NP or PA license or application in processActive and unrestricted DEA or application in processExcellent communication skills
