The Care Manager's role is to help children 0-21 years of age who struggle with complex medical, mental health, and/or psychosocial needs. In this field-based position, Care Managers work closely with the child and their families to assess clinical, care coordination, access issues, and support needs, and to partner with families to develop a patient-centered Individualized Plan of Care. This plan helps to integrate all of the child's clinical and support providers, and to identify and problem-solve for unmet needs.Job Responsibilities:Responsible for the creation and management of the patient's Individualized Plan of Care, driven by the patient needs.Through the Plan of Care, the care manager is able to:Conduct face to face visits for the patient and their caregiver(s) on a monthly basis per their acuity level within the 5 boroughsConduct check-ins and coordination with families and other service providers by phone, email, and telehealth, as neededSupport adherence to treatment recommendations through check-ins, appointment reminders, facilitating transportation needs(e.g. Arranging medicaid transportation), following up after ER visits, problem-solving for medication access barriers,etc.Monitor and evaluate a patient's needs, including prevention, wellness, medical, mental health, care transitions, and social and community resources where appropriateMeet care management documentation requirements in a timely and accurate manner by effectively utilizing designated electronic record and communication management systemsPromote wellness and prevention by linking patients with resources and services, based on their individual needs and preferences.Identifiy available community-based resources and actively manage referrals, access, engagement, follow-up, and coordination of servicesAttend and participate in ongoing staff development trainings to enhance skills needed to effectively meet the demands of the Care Manager positionAdvocate for patients within the agency and with external providersResponsible for completing the Child and Adolescent Needs and Strengths (CANS) assessment, crisis plan, information sharing consents, and other documents as required by the NYSDOH and other oversight agenciesThe New York Foundling is committed to attracting and retaining a diverse employee population,the Foundling will honor your experiences, perspectives and unique identity. Together, our community strives to create and maintain working and learning environments that areinclusive, equitable and welcoming.Requirements:Bachelor of Arts or Science in Psychology, Sociology, or Social Work with two years of relevant experience ORA license as a Registered Nurse with two years of relevant experience OR a Masters in Nursing and one year of relevant experienceWorking knowledge of the provision of health care in a variety of settingsAbility to work directly in the field (homes, schools, health centers) with families with complex needs, including children with chronic medical conditions, Medically Fragile, Developmental Delay, Severe Emotional Disturbance, placement in foster care, and histories of trauma.Ability to travel to assigned operational areas/facilities and patients homes within the 5 boroughs regularly, in combination with remote work and time in the New York Foundling central office in ManhattanAbility to have a flexible working schedule to accomodate working familiesDemonstrated strong commitment to safetyBachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution$52,530 (plus $3,800 education additive and $3,500 language additive)
