Position Details Unit C5 Locals NOT accepted - Local is perm residence within 50 miles of facility ASAP Starts Requested Dysrhythmia exam needed (study guide available in Simplify library) Strong Ortho exp/background required Certs: BLS, ACLS, NIH Shift: 12 Hour PM Unit Specific Details: Unit C5 Bed amount: 30 Patient population: Medical u2013 Surgical, ortho surgeries u2013 total joint replacements, TIA, Stroke workups Drips: Lasix, Heparin Lines/Drains/ Specific devices used: Wound vacs, JP drains, colostomies, Nephrostomy tubes, NG Tubes, rectal tubes, suprapubic catheters, gtubes, jtubes, Central lines. Oxygen: Nasal Canula, High Flow Canula, Bipap at night only, Non rebreather, venturi mask, face mask with oral surgeries, capnography with PCAs. Support staff (if applicable): PCTs, HUC Currently Staffing - BSMH - AVPA & HAA agreement Assignment Details: - VMS - Simplify - Acute RNs - 36 hour contracts, 12 hour shifts, 7-7 Orientation Information Orientation takes place on a bi-weekly basis every other Monday at 8am. Staff will begin at the offsite testing location where they will complete general onboarding and receive access to their login/timekeeping profiles Candidate Must Be EPIC Proficient Submittal Requirements Cover page (BSMH cover page) - attachments v traveleru2019s name, start date, preferred interview times, best number to call, requested time off v Resume/Work History v Must go back at least 5 years and have at least 1-year of experience in the specialty they are applying for (please see job notes for any changes to experience required) v Skills Checklist v List of Certifications required for position v 1 Supervisor Reference v Copy of Nursys License or RN license verification (if available) v Driveru2019s License to confirm compact licensure (if applicable) No longer needed for submission but will be required to send in when offer has been accepted v Candidateu2019s Address, Phone Number, Email Address v Full SSN & DOB (MM/DD) u2013 masked and encrypted to be entered in Workday Payroll / Billing info Bill Rate: Refer to Shiftwise Assignment Unbilled Orientation: 24 hours OT x1.3 after 40 hours Holiday x1.5 from 11pm-11pm Call Back x1.5 (overrules OT rates) Work Order Confirmation be drafted (template found under attachments) Credentialing Requirements See the attached Simplify / Trustaff Credentialing Checklist On Call Point of Contact - Nurse Manager Office @ 757-398-2213 Cancellations At least ninety (90) minutesu2019 notice must be given by Agency for all shift cancellations or changes. If Agency fails to provide at least ninety (90) minutesu2019 notice, Hospital System will be credited ninety (90) minutes of time. Hospital System may cancel up to twelve (12) hours in a pay period. 90-minutesu2019 notice must be given by Hospital System for all shift cancellations or changes. Failure to provide proper notice will result in 90 minutes of time billed to Hospital System at the Agency Personnelu2019s bill rate.
