LPNs work under the supervision of the Director of Nursing (DON) to provide hands-on care, support personal care services, and ensure client safety and satisfaction.Key Responsibilities- Provide direct nursing care in accordance with individualized care plans- Administer medications and treatments as delegated by the RN/DON- Monitor client conditions and report changes promptly- Document care and observations accurately and timely- Assist with ADLs, personal care, and mobility support as needed- Collaborate with RNs, STNAs, HHAs, and administrative staff to ensure continuity of care- Support PASSPORT and Personal Care services in compliance with OAC 173-39Qualifications- Current Ohio LPN license in good standing- 1+ year of experience in home health, long-term care, or community-based services preferred- Strong communication and documentation skills- Ability to work independently and as part of a team- Must pass background check and drug screeningSchedule & Work Environment- Flexible scheduling (day, evening, and weekend shifts available)- Local travel for home visits within Franklin County and surrounding areas- Supportive leadership and opportunities for advancementHow to ApplyEmail your resume to: Call: 614-585-8008 or 614-585-8006Visit: [
