Job Description

Primary Job Functions:Clinical ResponsibilitiesDemonstrates the ability to assess recipient needs and strengths utilizing the CANSIdentifies individualized goals and objectives and plans for appropriate servicesMonitors recipients to secure placement in the least restrictive environmentProvides ongoing monitoring of recipients’ progress and needs as assessed in the CANSAdvocates on behalf of the recipients to secure community resourcesEffectively collaborates with the Nova Mobile/Nova Washington First Episode Psychosis Program treatment team members and outside sources in the delivery of case management services to ensure goals are being achievedEnsures that services provided are meaningful, respectful and promote self-determinationEffectively demonstrates the application of youth/young adult specific competenciesParticipates in the development of crisis intervention and support plans for each participantAssists with meeting the needs of participants by facilitating their ability to live independently, obtain employment, achieve educational goals, and/or function in the community more effectivelyProvides transportation or links to transportation services when necessaryProvides psycho-educational services and co-leads multi-family groupsPerforms other duties as a Nova Mobile/Nova Washington First Episode Psychosis Program team member as assigned by the supervisorSupervision and ConsultationSeeks clinical supervision and consultation as neededAccepts and employs suggestions for improvementActively works to enhance case management skillsClinical Record KeepingDocuments in a timely fashion per AltaPointe policyDocuments appropriate, complete, and comprehensive CANS assessments to identify recipient needs, strengths, and planned interventionsDocuments measurable and specific goals in the individualized service planDocuments in a clear and concise manner ISP goals addressed with each service providedMaintains current documentation of participant demographic information, releases of information, etc.Documents case disposition, case closings, follow-up and referralsCourteous and respectful attitudes towards recipients, visitors and co-workersTreats recipients with care, dignity and compassionRespects recipient’s privacy and confidentialityIs pleasant and cooperative with othersAssists recipients and visitors as neededPersonal values don’t inhibit ability to relate and care for othersIs sensitive to the recipient’s needs, expectations and individual differencesIs gentle and calm to recipients and familiesCase Load Management:Effectively manages caseload based on recipient needsMeets AltaPointe productivity standardsAdministrative and Other Related Duties as Assigned:Actively participates in Performance Improvement activitiesBachelor’s degree in a mental health discipline. Knowledge in youth/young adult specialty, outlined below, to be attained within six (6) months of employment:Knowledge of psychiatric disorders in the youth/young adult populationBasic knowledge of growth and development of youth/young adult recipients as related to all intellectual, emotional, spiritual, and physical aspects of their developmentKnowledge of developmental disabilities and mental retardation in the youth/young adult populationBasic knowledge of behavior management and parenting issuesKnowledge of cultural diversityKnowledge of behavior management, crisis intervention and interventions common to acute psychotic recipients, as well as, to non-violent crisis intervention practiceKnowledge of effects of physical/sexual/emotional abuse and neglect issuesKnowledge of available community resources as applied to youth/young adultCertified by the Alabama State Department of Mental Health to provide adult and/or youth/young adult case management servicesKnowledge of the CANS (Child and Adolescent Needs and Strengths Assessment)Experience facilitating behavioral health therapy groups with children, adolescents, and adultsValid Alabama’s driver’s license
