Summary: Provides pharmaceutical care according to professional standards and practices and complies with all Federal and State regulations in the practice of Pharmacy at Virtua. Infectious Diseases Clinical Pharmacy services to include serving as the Pharmacist Co-Lead Antimicrobial Stewardship program along with the ID Physician Co-Lead for the health system. Serve as the committee lead for the Antibiotic Committee (P&T Subcommittee) and Antibiotic Stewardship System Committee. Collaboration with System Infection Control Committee and the infection control practitioners. Reporting data and metrics, both internal and external to Virtua. Performing audits and review that will lead to process improvement and better patient outcomes. Additional responsibilities include working on system-wide guidelines and protocol standardization and education for the pharmacy department and other clinical departments as needed. Participation in Pharmacy & Therapeutics Committee, in addition to precepting pharmacy residents and pharmacy students. Virtua is a five hospital health system which includes patient populations such as mother-baby, pediatrics, cardiac surgery, abdominal transplant, and neuro / neurosurgery, critical care, and surgical groups (for example: colorectal, orthopedic, bariatric, obgyn, thoracic). Position Responsibilities: • Participates in patient rounds with appropriate individuals including physicians, nurses and other health care providers. • Evaluates medication prescribing patterns and identifies opportunities for improving drug therapy. • Drug Use Evaluations • Provide education to patients/families, and other health care practitioners. • Participate in medication safety initiatives • Works within key sub-specialty areas (ex. Infectious Diseases and sub-specialties) to develop and maintain specific routine monitoring criteria for these patients. • Represents the Pharmacy Dept. in system wide and divisional committees as needed. Required Education: Graduate of an accredited College of Pharmacy with Doctor of Pharmacy (PharmD) degree or BS and comparable clinical experience. PGY-2 in Infectious Diseases Preferred Training/Certifications/Licensure: Active New Jersey pharmacy license.
