Case Manager (Caregiver, Lizzy, Alz.)/HVCC Screener (support) Job Description Position title Department Reports to Case Manager (Lizzy, Caregiver, Alz) HVCC Screener (support) Coordinated System of Service Executive Director Employment status FLSA status Effective date Full-time Part-time/Per Diem Nonexempt Exempt 12/01/2025 Summary: (CM): This position requires community-based activities (home visits, home assessments and outreach to caregivers). Input data to track appointments, coordinate community/volunteer support, provide caregiver supportAlzheimers) and document units of service. Position does not have supervisory responsibilities. (KCC): Assist with screening applicants for KidsConnect Summer Camp (IDD). (HVCC): Assist with screening applicants for HVCC and make referrals for needed services. This position does not have supervisory responsibilities. (KCC): Program runs in August. Duties and Responsibilities: Other duties may be assigned. 1. (CM): Represent Jewish Family Service at Community Meetings such as: a. Long Term Care Council b. Alzheimers PAC c. RJFS Program meetings, etc. 2. (CM): Billing for grants RJFS, Office for the Aging Title III-E Caregiver Grant, and CSE CM Other. 3. (CM): Provide assistance to participants/neighbors requesting services, enter requests into Volunteer Hub and/or E-Tapestry. 4. (CM)(KCC): Connect routinely with participants/neighbors, caregivers, families, staff, and volunteers to ensure satisfaction with services. 5. (CM)(KCC): Evaluate participants/neighbors for the program to determine needs of participant/neighbor and refer when necessary to alternative/additional services. 6. (CM)(HVCC)(KCC): Document all contacts in E-Tap Journal and Volunteer Hub databases for SIP as well as utilize UniteUs database for HVCC evaluations, referrals, and screenings. 7. (CM): Refer recipients to services, benefits and entitlements available in the community. 8. (CM): Check VolunteerHub, E-Tapestry, personal phone line, manage voicemail messages and emails daily. 9. (CM)(HVCC)(KCC): Make referrals for HVCC, Financial Empowerment, case management, intake, counseling, and developmental disabilities programs when necessary. 10. (CM)(FE): Maintain participant/neighbor records electronically including but not limited to uploading all supporting documents, ensuring neighbor remains active in the capacity and location of their choosing, deactivating neighbor when they are no longer receiving a service. 11. (CM)(HVCC)(KCC): Evaluate all participants/neighbors for appropriateness of services requested in accordance with the funding source of the program each participant/neighbor is participating in. 12. (CM): Prepare monthly reporting as needed. 13. (CM)(KCC): Responsible for optimized program utilization of the participant/neighbor database as trained, ensuring full understanding and supporting participants when necessary. 14. (CM): Review and analyze statistical and program data in relationship to the raw data of new/present/lost neighbors/participants. 15. (CM): Provide relevant reports for programs weekly/monthly/quarterly/annually to Director and as needed. 16. (CM): Provide program support for SIP team to maximize the development of programs and assist those participating. 17. (CM)(HVCC)(KCC): Support Database Manager and Outreach Coordinator by conducting intakes and assessments of participants/neighbors as needed. 18. Maintains ongoing representation of agency through participation in advisory meetings and collaborative relationships in the community as needed.
