This vital roleis responsible forguiding patients through the complexoncologycare continuum—from diagnosis through survivorship—by ensuring seamless coordination, communication, education, and support. The Nurse Navigator works closely with physicians, nurse practitioners, and other interdisciplinary team members to proactivelyidentifyhigh-risk patients, coordinate care, and improve outcomes through patient-centered navigation services. Key Responsibilities: Care Coordination & Patient Navigation: Identifyhigh-risk oncology patients and provideproactive outreach and follow-up, including in-person and telephone communication. Collaborate with physicians and care teams to develop individualized care plans, coordinate follow-up appointments, scans, and diagnostic tests. Ensuretimelycommunication and coordination between the patient, family, and care team, acting as a primary point of contact. Serve as aliaisonbetween referring physicians and the oncology team to ensure streamlined transitions in care. Monitor treatment progress, compliance, and follow-up needs; support patients in understanding their treatment plans and care pathways. Coordinate medication refills, imaging, lab orders, and necessary authorizations in partnership with the provider and financial coordinator. Patient Education & Support: Provide anticipatory guidance and education for patients withchronic diseasesor new diagnoses, emphasizing self-management and care ownership. Teach patients and families how to schedule scans, arrange transportation, and navigate the healthcare system. Integrate patient and family education throughout all phases of care, adapting to individual learning needs and cultural considerations. Clinical Support & Communication: Triage abnormal lab results and clinical messages for providers. Manage prescription refill requests and clinical inquiries. Support providers in addressing urgent patient issues and care coordination. Program Development & Quality Improvement: Participate inquality improvement initiativesto enhance patient outcomes and satisfaction. Contribute toaccreditation effortsand the ongoing development of the cancer program through data collection, education, and process improvement. Maintain awareness of and adherence to regulatory requirements, hospital policies, and best practices in oncology nursing.
