Job DescriptionJob DescriptionCommunity Connections of Franklin CountyJob DescriptionParent and Child MentorJob Summary:This position is a full-time position consisting of 40 hours per week. The Parent and Child Mentor will collaborate with other agencies to offer a variety of mentoring services to referred children, parents, and families.Qualifications:High School Diploma or GED required, associate degree within a Human Services field preferredBe able to review, assist and refer parent and child(ren) on the social determinants of health which include, but are not limited to, primary care; behavioral health; dental; environmental; housing; financial; employment and social.Can maintain appropriate professional boundaries.Ability and willingness to work nights and weekends along with regular weekday hours.Ability to establish and maintain rapport.Must be a parent.NYS Peer Certification within 6 months of employment.Personal experience advocating a variety of issues in Franklin CountiesStrong interpersonal skillsStrong communication skillsStrong active listening skillsAbility to multitask in a fast-paced environment and collaboratively with a team of professionals across various agencies and skill disciplinesStrong computer skills working with word and excelReliable transportationWilling to travel 90% or more a weekActive drivers LicenseUp to date car insurance, registration, and inspectionStrong organizational & time management skillsAble to pass a NYS Criminal Justice Background CheckNot listed on the Staff Exclusion ListAble to clear an OCFS BackgroundAbility to make decisions within the guidelines of the agencies policies & procedures, federal and state lawsJob Responsibilities:Must complete Mandated Reporting and HIPAA training within the first week of employmentMust obtain New York State Peer Specialist Certification within 90 days of hire dateMust obtain an additional 10 hours of CEUs annually after receipt of New York State Peer Specialist Certification to maintain credentialsKeep statistical data to report to their direct supervisor by the last working day of each monthMust have knowledge of resources and systematic processes in Franklin CountyMust continually learn systemic processes for all systems in Franklin County such as, but not limited to, courts, probation, Department of Social Services, Public Defenders, Conflict DefendersMust abide by all agency policies and proceduresMust maintain confidentiality of individuals worked with and agency functionsResponsible for assisting with daily upkeep and cleaning of the agency, including copier and fax machineEngage in supervision meetings monthly with their direct supervisor.Help to connect individuals to providers they deem suitable for their overall well-being such as but not limited to, primary care, etc.Empower families to resolve current issues and to develop coping strategies for the future.Provide a model for a healthy, trusting relationship through clear communication and setting appropriate boundaries.Acknowledge parent, child and family strengths, talents and gifts and encouraging them to find ways to use them.Be an active listener and offer support and encouragement when appropriate.Engage in activities outside of the home to help expose parent, child(ren) and families to new opportunities and experiences.Ability to focus on the root cause of issues and not just the issue presentedComplete continued education on recovery to help individuals you are working withMaintain a professional working relationship with all agencies on behalf of CCFCMaintain a professional attitude with all agencies CCFC collaborates with regarding trainingComplete all paperwork per agency guidelinesMust promote community integration and involvementComplete in-house referralsSupport/work in all programs on a temporary basis to meet the needs of those receiving services as needed.Any other job duty as assigned by direct supervisor.
