This role is not a general therapy position it's ideal for licensed professionals with at least 4 years of post-master's experience conducting comprehensive diagnostic assessments for clients entering mental health or substance use treatment.You'll play a vital clinical gatekeeping role by assessing risk, determining service appropriateness, and shaping care pathways for children, adults, and families across our Article 31 and Part 822 clinics. A diagnostic and intake specialist role that evaluates clients for care ? Focused on clinical formulation, risk assessment, documentation, and handoff ? Not a fee-for-service therapy caseload ? Not a general outpatient therapist positionKey ResponsibilitiesConduct in-depth intake assessments for new clients (in-person and Open Access walk-ins)Formulate DSM-5 diagnoses and determine program/service appropriatenessUse clinical tools (e.g., PHQ-9, CSSRS, GAD-7) to assess risk and guide treatment planningPrioritize high-risk intakes and coordinate smooth handoffs to assigned therapistsCollaborate closely with prescribers, supervisors, and support staffRefer to community resources when clients are better served elsewhereMaintain timely, accurate documentation in our EHR (CareLogic)Provide short-term support to clients awaiting clinician assignment when neededCarry a small caseload or facilitate therapeutic groups as neededParticipate in staff training, QA, and team meetingsWho We're Looking ForLMSW requiredLCSW preferred (NYS licensure only)4+ years of clinical experience with intake, diagnostics, and risk assessmentStrong knowledge of co-occurring disorders, including substance use and gamblingExperience with children, adolescents, and adults across diverse communitiesSkilled in evidence-based models like CBT, DBT, MI, etc.Deep familiarity with DSM-5 and behavioral health intake workflowsComfortable with EHR documentation, Microsoft Office, and compliance standardsKnowledge of OMH Part 599 and OASAS Part 822 regulationsBilingual (e.g., Spanish or Bengali) a plusWhat We OfferFull-time, salaried position with hybrid flexibilityStructured schedule with no FFS pressureMeaningful role helping shape care access and triageComprehensive benefits package for full-time staff: Medical, dental, vision insurance; Life and long-term disability coverage; Free parking and commuter benefits; Employee Assistance Program; Professional development support; Profit-sharing planIf you're a clinically experienced, assessment-driven social worker ready to lead clients into the care they need - we want to hear from you.Apply today and be the front door to mental health and recovery for our Queens community.
