Job Title: LTSS Coordinator - Case Management (BA)Location: New York, NY 10004Pay Rate: $37 - $41/hourSchedule: 35 hours/week (7 hours/day, Monday-Friday)Contract Duration: 05/25/2026 - 07/18/2026 (Extendable)Position OverviewWe are seeking a dedicated LTSS Coordinator to support care management services for Medicaid members receiving long-term services and supports. This role works closely with the LTSS Manager and Care Manager to coordinate care, improve health outcomes, and ensure members have access to appropriate healthcare services.The ideal candidate will have strong communication skills, experience in care coordination, and the ability to manage multiple cases while maintaining compliance with regulatory standards.Key ResponsibilitiesConduct monthly outreach to assigned members while ensuring HIPAA complianceComplete LTSS assessments and Social Determinants of Health (SDOH) evaluationsCoordinate member care needs including appointments, transportation, medications, and durable medical equipment (DME)Connect members with appropriate healthcare providers and community resourcesAccurately document all interactions in care management systems (e.g., DCMS)Escalate clinical concerns to care managers as neededSupport care team operations including referrals, reporting, and administrative tasksEngage members and caregivers to promote self-management and wellnessIdentify and address gaps in care and utilization issuesMaintain strict confidentiality and compliance with organizational policiesAssist in data collection, reporting, and quality improvement initiativesCollaborate with cross-functional teams such as customer service, utilization management, and provider relationsParticipate in audits, training sessions, and organizational development activitiesPerform additional duties and special projects as assignedRequired QualificationsBachelor's degree requiredStrong communication and interpersonal skillsAbility to manage multiple cases and prioritize effectivelyExcellent organizational and documentation skillsAbility to work independently and escalate concerns appropriatelyPreferred QualificationsExperience in care coordination, case management, or healthcare support rolesFamiliarity with Medicaid, LTSS programs, or case management systemsExperience working with diverse populations and underserved communitiesBenefits:Pride Health offers eligible employees comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), 401(k) retirement savings, life & disability insurance, an employee assistance program, legal support, auto and home insurance, pet insurance, and employee discounts with preferred vendors.
