Physical Therapist

Pride Health is hiring a Physical Therapist to support our client's medical facility in Richmond, KY 40475. This is a 3+ month contract assignment (convertible to permanent based on your work performance). and a great way to start working with a top-tier healthcare organization! Specific Duties:Perform comprehensive patient evaluations and develop individualized plans of careProvide skilled physical therapy services in patients' homesConduct Start of Care (SOC), Resumption of Care (ROC), recertifications, and discharge visitsMonitor patient progress and adjust treatment plans as neededEducate patients and caregivers on exercises, safety, and ongoing careMaintain timely and accurate clinical documentation in compliance with regulationsCollaborate with physicians, nurses, and interdisciplinary team membersEnsure compliance with Medicare guidelines and home health regulationsQualifications: Degree in Physical Therapy (PT)Active Physical Therapist license (or eligibility) in the state of ConnecticutPrior home health experience preferredStrong understanding of patient care, treatment planning, and documentationExcellent communication and organizational skillsAbility to work independently and manage a flexible scheduleMust have qualifications/experience:NOTE: for LHC GL coding guidance, use the LHC Group Contract Labour GL Coding Guide, available on the LHC Group Home Page.Familiarity with home health EMR systems (e.g., HCHB)Previous travel or contract experience BLS for healthcare providersHigh school graduate or GED equivalent1+ years of experience as a Physical Therapist Additional Information: Location: Richmond, KY 40475Job Type: 3+ months contract to hireSchedule: Monday through Friday Shift start & end times: Monday-Friday, 8:00am - 4:30pmWorkdays per week: 5 daysPay Range: $50-57/hrs. on W2 without benefitsPHCUHGClinical $250 Offered pay rate will be based on education, experience, and healthcare credentials.
