Coverage Areas: Madison, Montgomery, Clark, Powell Schedule: Monday - Friday, 8:00 AM - 4:30 PMPosition OverviewWe are seeking a compassionate and skilled Physical Therapist to join our Home Health/Hospice team. In this role, you will provide high-quality, patient-centered care in home settings, helping patients improve mobility, safety, and overall quality of life.Key ResponsibilitiesConduct comprehensive in-home physical therapy evaluations, including assessment of mobility, strength, balance, pain, endurance, and safetyDevelop, implement, and update individualized Plans of Care (POC) based on physician orders and patient goalsDeliver skilled physical therapy interventions to improve mobility, gait, balance, strength, and pain managementEducate patients and caregivers on diagnosis, fall prevention, home exercise programs, and use of medical equipmentPerform home safety assessments and recommend modifications to reduce fall riskMaintain timely, accurate, and compliant documentation (evaluations, progress notes, reassessments, discharge summaries)
