Home Infusion Pharmacist - Irving, TexasA leading independent home infusion provider in the U.S. is seeking a dedicated and experienced Pharmacist to join its collaborative team in Irving, TX. This is a full-time role offering a competitive hourly wage and the opportunity to make a meaningful patient impact in the home setting.Pay Rate: $62 - $69 per hourSchedule/ShiftThe standard hours are 9:30 AM to 6:00 PM. When on the on-call rotation, the shift changes to 10:30 AM to 7:00 PM.On-Call Rotation: Every 7th weekend and approximately three weekdays per month. The weekday on-call days are coordinated with the team and serve as a "late day" that replaces the regular shift.Key ResponsibilitiesAssess patient suitability for home infusion therapy.Design, monitor, and evaluate individualized pharmacy care plans.Recommend or adjust medication therapies based on clinical data.Prepare, compound, label, and dispense medications per USP and DEA standards.Conduct nutritional assessments and provide patient/caregiver education.Collaborate with nursing and other departments to ensure continuity of care.Monitor controlled substance inventory and report discrepancies.Provide drug information to healthcare professionals and patients.Participate in Continuing Education Unit (CEU) and professional competency programs.Adhere to company policies, quality assurance, and regulatory requirements.Required Skills and QualificationsGraduate of an accredited School of Pharmacy.Active Pharmacy License in the State of Texas (TX).Must have infusion experience.One of the following:1+ year experience in sterile IV admixture procedures.Knowledge of non-sterile compounding, immunizations, and MTM.Completion of ASHP-accredited Pharmacy Residency Program.Completion of an Advanced Pharmacy Practice Experience.Equivalent experience approved by VP/SVP of Pharmacy.Proficient in Microsoft Word, Excel, and PowerPoint.Strong customer service and patient-care orientation.Preferred SkillsExperience in home care, hospital, or clean room settings.ASHP-accredited Pharmacy Residency completion.1+ year of indirect management experience.
