Full time & Part time availabilityJob Title: Nurse Practitioner Primary Care (Long-Term Care & Wound care)Location: Multiple sites across Inez, KY Schedule: Monday-Friday | 8:00 AM - 5:00 PMPosition overview: We are seeking a Nurse Practitioner to provide wound care and primary medical services to residents in skilled nursing and long-term care facilities. This role focuses on delivering high-quality, patient-centered care while collaborating with interdisciplinary teams and facility staff to manage complex and chronic conditions.Key ResponsibilitiesDeliver wound care and primary care services to long-term care residentsAssess, diagnose, and treat acute and chronic conditionsPrescribe medications, including controlled substances, in accordance with Ohio lawOrder, review, and follow up on diagnostic tests and treatmentsStage pressure injuries according to NPIAP guidelinesManage complex wounds (diabetic, arterial, venous, mixed etiology, Kennedy ulcers)Utilize advanced wound care products (collagens, alginates, foams, silver dressings, etc.)Use wound vacs and pressure-relief equipment as appropriateCommunicate regularly with nursing staff regarding patient status and care plansCollaborate closely with primary care physicians and interdisciplinary teamsDocument visits and care plans accurately within EMR systemsRequired QualificationsActive, unrestricted Nurse Practitioner license in OhioBoard CertificationClinical background in adult, family, or geriatric careExperience treating chronic or complex woundsAbility to manage patients with multiple comorbiditiesExperience in long-term care settingsProficiency with EMR systemsStrong collaboration and communication skillsCompensation & BenefitsSalary: $115,000 - $130,000Mileage reimbursementMalpractice insurance coverageHealth, dental, and vision insurance401(k) with employer matchPaid time offContinuing education creditsRecertification reimbursement
