Medical Social Worker (LMSW/LCSW)We are seeking a compassionate and motivated Medical Social Worker to join a fast-paced hospital setting supporting patient care and discharge planning. This role is ideal for an LMSW or LCSW who is comfortable working directly on a hospital unit alongside a multidisciplinary team. New LMSWs are encouraged to apply if they are confident working in an acute care environment rather than a traditional office setting.ResponsibilitiesProvide social work support to patients within a hospital/unit settingManage discharge planning for approximately 25 patients per dayCollaborate closely with Nurse Care Managers (NCMs), physicians, and interdisciplinary teams to coordinate safe transitions of careAssess patient psychosocial needs and identify appropriate community resources and support servicesDocument interventions and maintain accurate patient recordsParticipate in multidisciplinary treatment planning and care coordinationOpportunity to work within pediatric populations if interestedRequirementsActive LMSW required (LCSW strongly preferred)Comfortable working in a hospital or inpatient settingStrong communication, organizational, and care coordination skillsAbility to manage a high patient volume in a fast-paced environmentPreferred QualificationsMinimum of 2 years of experience in medical social work, mental health, or human services as part of a multidisciplinary treatment teamPrior hospital or inpatient experience strongly preferredCandidates with outpatient or nonprofit-only backgrounds may not be the best fit for this roleScheduleMonday - Friday8:30 AM - 4:30 PM or 9:00 AM - 5:00 PMThis is an excellent opportunity for a social worker looking to grow within a collaborative hospital environment while making a direct impact on patient outcomes and continuity of care.
