About the RoleRehab Center LLC is seeking a compassionate, clinically strong, and Speech-Language Pathologist to provide speech, language, communication, cognitive-communication, and swallowing-related therapy services to patients. This role is ideal for a licensed SLP who is comfortable working with diverse patient populations and can communicate effectively.The Speech-Language Pathologist will evaluate patient needs, develop individualized treatment plans, provide direct therapy, document clinical progress, and collaborate with physicians, therapists, families, caregivers, and other healthcare professionals to support patient outcomes.Key Responsibilities • Conduct speech, language, communication, cognitive-communication, voice, fluency, and swallowing evaluations, as appropriate. • Develop individualized treatment plans based on clinical findings, patient goals, physician referrals, and plan-of-care requirements. • Provide direct therapy services to patients in accordance with professional standards and applicable regulatory guidelines. • Communicate treatment goals, expectations, and progress clearly to patients, families, caregivers, and interdisciplinary team members. • Maintain accurate, timely, and compliant clinical documentation in the required documentation system. • Monitor patient progress and adjust treatment plans as clinically appropriate. • Educate patients and families on home programs, therapy exercises, communication strategies, and continuity-of-care recommendations. • Collaborate with occupational therapists, physical therapists, physicians, case managers, and administrative staff. • Support a culturally competent care enviroment. • Maintain compliance with applicable licensure, credentialing, privacy, and documentation standards.Qualifications • Degree in Speech-Language Pathology, Communication Sciences and Disorders, or a related accredited program or related program with certification. • Active and unrestricted Speech-Language Pathologist license • Strong clinical assessment, treatment planning, documentation, and patient-care skills. • Ability to work with patients, families, and interdisciplinary care teams. • Strong communication skills, professionalism, and patient-centered bedside manner. • Ability to manage caseload priorities and maintain timely documentation.
