OverviewWe are seeking a compassionate, organized, and compliance-minded Care Manager to support older adults and their families throughout the Los Angeles area. This role ensures that seniors receive high-quality, person-centered care through assessment, care planning, coordination, and ongoing monitoring. The ideal candidate understands California senior-care regulations, maintains meticulous documentation, and thrives in a field-based environment.Key ResponsibilitiesCare Coordination & Client SupportConduct in-home assessments to evaluate physical, cognitive, emotional, and environmental needsDevelop individualized care plans aligned with client goals, safety needs, and regulatory requirementsCoordinate services such as home care, medical appointments, transportation, therapies, and community resourcesMonitor client progress through regular visits, check-ins, and reassessmentsAdvocate for clients and support families in navigating care decisionsCompliance & DocumentationMaintain accurate, timely, and compliant documentation in accordance with:California Department of Social Services (CDSS) guidelinesHIPAA privacy and security standardsHome Care Organization (HCO) regulations (if applicable)Medicare/Medicaid documentation requirements (when relevant)Ensure care plans, incident reports, and service notes meet internal and external audit standardsIdentify and report suspected elder abuse or neglect per California mandated reporter lawsParticipate in quality assurance reviews and compliance auditsCollaboration & CommunicationWork closely with caregivers, nurses, physicians, social workers, and community partnersProvide guidance and support to care teams to ensure consistent, high-quality service deliveryCommunicate clearly with families regarding care updates, changes, and recommendationsQualificationsRequiredBachelor's degree in Social Work, Nursing, Gerontology, Psychology, or related fieldMinimum 2-3 years of experience in senior care, care management, case management, or social servicesStrong understanding of California senior-care regulations, including:HIPAACDSS Home Care Services Consumer Protection ActMandated Reporter requirements (Elder Abuse and Dependent Adult Civil Protection Act)OSHA safety standardsExcellent documentation, assessment, and organizational skillsValid driver's license and reliable transportation for field visitsAbility to pass background checks required by California HCO regulationsPreferredMaster's degree in Social Work (MSW), Gerontology, or related fieldCertification such as:CCM (Certified Case Manager)CMC (Care Manager Certified)CDP (Certified Dementia Practitioner)Experience with dementia care, chronic disease management, or behavioral healthBilingual skills (Spanish, Korean, Mandarin, or other languages common in Los Angeles communities)Core CompetenciesEmpathy and cultural sensitivityStrong problem-solving and crisis-management abilitiesKnowledge of local senior-care resources and community programsAbility to work independently and manage a dynamic caseloadHigh ethical standards and commitment to complianceWork EnvironmentHybrid role with in-home visits, community outreach, and administrative workOccasional evening or weekend availability based on client needs
