Position Summary:The Member Services & Program Coordinator plays a key role in ensuring a welcoming, supportive, and effective experience for members and families at our adult day program and adult transition program. This position serves as the first point of contact for prospective families, guides them through the admissions process, and ensures all members have high-quality, person-centered service plans and goals. The ideal candidate is highly organized, warm and professional in communication, and experienced in coordinating care for neurodivergent individuals or those with complex support needs.Key Responsibilities:Family & Member LiaisonConduct tours for prospective members and families, providing clear and compassionate guidance throughout the intake processServe as the primary point of contact for family communication and supportLead onboarding of new members, ensuring a smooth transition into the programAdmissions & PlanningReview and approve applications for new members based on program criteriaDevelop and write person-centered service plans and SMART goals tailored to each member's needs and strengthsInput and manage member goals in the agency's electronic documentation system (e.g., Therap)Program Support & OversightOrder, approve, and track materials and supplies needed to implement individual goal programsCoordinate and lead family meetings to review member progress, update plans, and strengthen collaborationAdminister medications as needed according to agency policy and licensing guidelinesStaff Collaboration & TrainingCollaborate with coaches and interdisciplinary team members to ensure goal implementation and progress trackingSupport training and onboarding of new coaches to ensure fidelity to programming and values-based careQualifications:Bachelor's degree in human services, psychology, special education, nursing, or related field (Master's preferred)Experience with person-centered planning, case coordination, or working with adults with developmental disabilitiesFamiliarity with documentation platforms like Therap or similar systemsExcellent organizational, communication, and interpersonal skillsComfortable navigating both clinical and relational aspects of member careCertified or willing to be trained in medication administration per licensing standardsJob Type: Full-timePay: $75,000.00 - $90,000.00 per yearBenefits:401(k) matchingDental insuranceFlexible scheduleHealth insuranceVision insuranceSchedule:8 hour shiftMonday to FridayAbility to Commute:Oceanside, CA 92056 (Required)Ability to Relocate:Oceanside, CA 92056: Relocate before starting work (Required)Work Location: In person
