Tittle: Professional - Coordinator, Clinical Cancer Genomics (CCG)Location: 1500 E Duarte Rd., Duarte, CA 91010Duration: 12 MonthsPosition Summary:The clinic coordinator under the general direction of the Practice Manager provides research support to the Division Chief and Associate Director of the Division of Clinical Cancer Genomics and the entire clinical care team of genetic counselors and nurse practitioners who provide genetic cancer risk assessment for patients at Hospital (Duarte, Irvine and clinical network).The coordinator will assist in the appropriate triaging of patients, schedule/reschedule patients referred to Genetics into the appropriate clinic, obtain any necessary patient information prior to an appointment, ensure that necessary follow-up appointments are scheduled and completed, as well as assist with ongoing communication with patients and referring healthcare providers.The coordinator will also play a critical role in managing point-of-care services. Essential Functions:Schedules and/or reschedules patients into the appropriate Genetics clinic while ensuring clinician template utilization and optimization and accommodating patient preferences.Facilitates care coordination for patients in the enterprise-wide clinic, Duarte, and Lennar through collaboration with each clinic's on-site coordinator and the COH clinical staff.Work with NPS to help coordinate authorization and scheduling of patients from the community sites at Duarte and patients who are new to COH.Coordinating various scheduling work queues (outbound, NPS, community sites) and moving orders as needed to the appropriate queue.Acts as the point person and coordinator for urgent consult requests.Sends orders for diagnostic tests to appropriate department. Assures that documentation indicating the date of service and the visit number accompanies the orders for diagnostic testing.Verifying receipt of completed family history, tests results (when indicated), and medical records as obtained by New Patient Services prior to the appointment and working to reschedule patients when necessary.Confirming before clinic if the patients are pre-authorized to be seen in Clinical Cancer Genomics.Receives and manages phone calls from patients and departments regarding care coordination and concerns related to patient care.Maintains and forwards CCG faxes to providers.Manage InBasket messages in Epic regarding non-patient care matters (scheduling, other inquiries).Manages the clinicians' clinic calendars; close clinics when out-of-office.Coordinate with scheduling to build or modify CCG providers template.Determine Triage GC schedule; mark it in the Division calendar.Minimum Education: Bachelor's Degree PreferredExperience may substitute for degreeMinimum Experience: 3 or more years of progressively responsible experience in a healthcare environment, the conduct of health science research or an equivalent combination of education and experience.Preferred Experience:Skills/Abilities: Bilingual in Spanish preferred.Excellent verbal communication and telephone skills.Excellent written communication skills.Medical TerminologyScheduling in EPICPossess a high level of administrative and secretarial skills and versatility in use of PC's and related office software.Excellent organizational skills.Displays tact/consideration in dealing with client`s patients and employees.Establish and maintain databases as needed.Interface with internal /external people at all levels.Handle multiple tasks simultaneously.
