This role is actively involved in assessments, care planning, admissions and discharges, and resident rights advocacy. The Social Services Coordinator collaborates with the interdisciplinary team to promote resident-centered care, maintain regulatory compliance, and foster a supportive community environment.Qualifications:Minimum of 2 years of social work experience in healthcare settings, with at least 1 year being in long-term care of skilled nursing, required.High School Diploma or GED, required.Bachelor's degree in Social Work or related field, preferred.Experience in a leadership role, preferred.Experience with MDS assessments and Medicare/Medicaid regulations, preferred.Comprehensive knowledge of federal and state regulationsStrong clinical assessment and intervention skills required.Excellent interpersonal and communication skills required.Demonstrated leadership capabilities, preferred.Benefits (dependent on eligibility):Excellent Health, Dental and Vision plansGroup Life Insurance - 100% company paidPet InsuranceEmployee Assistance ProgramGenerous PTO.403(b) Retirement PlanShift:Full-time, 40 hoursOn-Site
