We emphasize a patient-centered approach to health care and strive to deliver high-quality medical services.Annandale Family Medicine (AFM) In-Home Program (IHP) Medic Roles/ResponsibilitiesThis is a full-time hybrid role for a Medical Doctor. The Medical Doctor will be responsible for diagnosing, treating, and managing a wide variety of illnesses and medical conditions. They will perform routine check-ups, order and interpret diagnostic tests, prescribe medications, and provide preventive care. The role is primarily based in Annandale, VA, but some work-from-home tasks are acceptable.Practicing primary care medicine, including assessment and treatment of existing and/or new acute medical conditions, under the supervision of AFM providers.Initiating the chronic care phase once acute medical conditions are stabilized, which may include but not be limited to: chronic care management (CCM) initiation, social determinants of health (SDOH) identification, care gap closure, and quality measure achievement.Establish meaningful connections with patients and staff, ensuring all applicable teammates are engaged in the care processLeverage any and all applicable technology.Core Roles/ResponsibilitiesPatient Assessment & MonitoringPerform comprehensive home visits (vital signs, physical assessments, medication review).Identify early signs of deterioration in chronic conditions (e.g., CHF, COPD, diabetes, hypertension).Document and communicate findings to supervising providers.Preventive & Primary Care Support (including CCM and SDOH)Administer vaccinations, point-of-care testing, wound care, and basic labs (blood glucose, cholesterol, INR, etc.).Provide medication reconciliation and check for adherence or side effects.Educate patients and caregivers on disease management, nutrition, and lifestyle changes.Acute Care DiversionRespond to non-emergent complaints that would otherwise result in unnecessary ER or 911 calls.Stabilize patients at home when safe (e.g., dehydration, mild asthma exacerbation, minor infections).Coordinate with telehealth physicians for expanded scope and prescriptions.Care Coordination & NavigationAct as a liaison between patients, families, primary care, specialists, home health agencies, and social workers.Refer to community resources (behavioral health, housing, food support, addiction services).Help with post-discharge follow-ups to reduce readmissions.Mental & Behavioral Health SupportConduct mental health screenings (e.g., depression, anxiety, substance use).Provide crisis intervention and connect patients to behavioral health providers.Support adherence to psychiatric medication regimens.Documentation & Quality ImprovementUse EMRs to document visits, interventions, and care plans.Collect data for program evaluation (e.g., hospital readmission rates, avoided ER visits).Participate in quality assurance and training for continuous improvement.Typical Scope of Practice (Depending on Training & Oversight)IV access, blood draws, EKGs.Medication administration (per protocol and medical direction).Wound care and suture removal.Oxygen therapy, nebulizer treatments.Telemedicine facilitation with remote providers.QualificationsMedical degree (MD or DO) and valid state medical licenseExperience in diagnosing and treating a comprehensive range of medical conditionsStrong communication and interpersonal skills to effectively interact with patients and healthcare staffAbility to perform and interpret diagnostic testsProficiency in electronic health records (EHR) systemsCommitment to continuing education and staying updated with medical advancementsBoard certification in Family Medicine or Internal Medicine preferredCPR and Basic Life Support (BLS) certification
