With a growing senior population, we understand the importance of being present throughout the geriatric continuum, promoting rehabilitation, prevention, and wellness to facilitate positive outcomes and fiscal success.As a Speech Language Pathologist serving our Veterans and Gold Star Families, you will be responsible for providing quality direct patient care, including screening, evaluation, development of initial and ongoing treatment plans, discharge planning, and case management.DESCRIPTIONAssess residents to determine care needsOptimize residents' functionality and skillsComplete all necessary documentation via Net HealthAttend required meetings and in-services, including but not limited to family meetings, care plans, staffing, and utilization reviews (Not required for PRN staff)In conjunction with the DOR, develop programs appropriate to the needs of facility residents across the continuum of careRemain compliant with infection control and environmental protocols while following OBRA Resident Rights GuidelinesResponsible for appropriate scheduling of residentsFacilitate and complete family and facility staff in-services and educationAssist the facility in meeting standards necessary to maintain state and federal certificationComplete tasks deemed appropriate and assigned by the Director of RehabilitationServe Veterans and Gold Star Families with pride and commitmentREQUIRED SKILLSExcellent communication skills, both written and verbalAbility to maintain professionalism in all situationsEffective time management and the ability to manage a caseloadREQUIRED QUALIFICATIONSState licensure to practice as a SLP in South CarolinaTransPerfect Health is a proud DEI and EOE employer.
