RN Care Manager - In Home Acute Care / Hospital at Home Program This position will require travel around Madison, WI, where you will see 2-3 patients/day. Do you want to be part of an organization that has partnerships with some of the most well-known insurance companies as well as one of the most prestigious Hospital Health Systems in the local area?Do you have proven acute care experience and want to provide in-home services, while collaborating with clinicians who are experts in their field?If you answered yes to these questions, please consider applying today!Schedule:Full time, 4 10's per week, rotating every other weekend Compensation: $94,000-$105,000/year base salary, dependent on experience, plus an 8% bonusExtensive benefits package including 4 weeks' PTO, 401k company match plan, affordable healthcare coverage, mileage reimbursement, tuition reimbursement and moreRN Care Manager Responsibilities:Deliver acute-level care in patients' homes, including assessments, interventions and monitoringInteract with an interdisciplinary team to help coordinate in-home servicesEvaluate social determinants of health and incorporate them into care planningAdminister prescribed medications (typically intravenously), obtains blood samples, wound care, insert foley catheters and provides therapeutic interventionsManage biometric monitoring device and respond appropriately to data and alertsEducate patients, caregivers and family members on in-home best practicesHelp develop individualized care plans and ensure appropriate follow up through the care continuumRN Care Manager Qualifications: Associate's Degree in Nursing (RN), Bachelor's Degree in Nursing (BSN) preferredAn active Registered Nurse license in the state of Wisconsin (RN)Current CPR certification2+ year of acute care/hospital based experience (Med/Surg, ED, ICU, etc.), highly preferred1+ year of in-home case management / care coordination experience, preferredRN Care Manager - In Home Acute Care / Hospital at Home Program
