The Wellness Coach will assist consumers with accessing needed community services (medical, psychological, substance use, vocational, food, clothing, etc.) through the development of a referral network, referrals, and follow up.Essential Duties & Responsibilities:Provide case management to assigned caseloadDevelop and monitor individual service plans in conjunction with consumersAssist consumers with problem solving and crisis interventionAccompany consumers on medical, psychiatric, and public benefit appointments, as necessaryAct as advocate for consumers as appropriate; encourage and support self-advocacyIdentify problems in daily living skills; provide hands-on assistance and/or referral as neededSupport facilities staff in ensuring maintenance of apartment standards as required by fundersProvide supportive counseling to consumers as neededConduct regular home and hospital visits, as necessaryMaintain strong working relationships with all therapeutic support services to ensure quality of careDocument all services in compliance with program standardsAssist with moves with consumers and staff; may include lifting, packing, and cleaningWork as a member of the social service team to develop off-site recreation, socialization, educational, and skill-building activities to promote community inclusion and integrationServe as liaison to community-based services and groups, including civic, social service, government, and business organizationsParticipate in ongoing individual and group supervision, team meetings, case conferences, division meetings, and trainingsAdhere to on-call system to facilitate crisis intervention, emergencies, and other situations that require immediate actionPerform other duties as assignedQualificationsRequired Education and ExperienceBachelor's degree in Social Work or a related human services field with two years of relevant experience, or equivalent backgroundMinimum 2 years' experience working with individuals with mental illness, substance use issues, and/or a history of homelessness, or 2 years in the human services fieldExperience in group work, tenant organization, case management, referral, and/or community outreach strongly preferredExperience or training in peer counseling or advocacy preferredProficiency in common computer programs, including email, Microsoft Word, Excel, PowerPoint, and WindowsPreferred Qualifications and SkillsBilingual English/Spanish preferredPay: $50,000.00 - $55,000.00 per yearLocation: Bronx, NY (Onsite)
