Overview:We are seeking Nurse Practitioners (NP) and Physician Assistants (PA) to conduct initial health assessments (IHA) for Medicaid members in Georgia. The program goes live October 1st, and providers will play a key role in helping members receive timely, in-home care.This opportunity is flexible"”providers can commit a few days per week, evenings, or weekends, or work full-time to complete assessments quickly. Ideal candidates will have a background in home health and be comfortable traveling within a 50-minute radius of their home.Responsibilities:Conduct comprehensive histories and physical exams.Evaluate social determinants of health.Deliver health education on disease prevention, medication use, lifestyle changes, and self-care.Order diagnostics, develop and adjust treatment plans, and manage medications (as applicable).Complete and submit documentation within 24 hours of visit (primarily completed during the visit).Open Provider Needs by Location:Fulton County, GAQualifications:Active NP or PA license.Minimum 1 year of experience.Ability to complete 30 assessments within 1 month.Why Join:Flexible scheduling - nights and weekends available.Extra income opportunity.Potential for additional longitudinal home care work after initial project.
