We prioritize dignity, quality, and continuity of care for every patient we serve.Role DescriptionWe are seeking a Licensed Vocational Nurse (LVN) or Medical Assistant (MA) to join our team in Richardson, TX. This is a full-time on-site role supporting in-home primary care services for seniors and adults with chronic conditions.The LVN or MA will work closely with our physician to provide clinical support, care coordination, and patient communication.Key ResponsibilitiesMedication management, reconciliation, and refillsReview and follow-up of labs, X-rays, and EKG results (LVN)Handling clinical phone calls and communicating with patients/familiesPerforming diagnostic procedures (ABI/ANS testing) as assigned - at least once a weekHandle Chronic Care Management (CCM) & Remote Patient Monitoring (RPM) programsPerforming Annual Wellness Visits (AWVs) and patient assessmentsProviding care coordination, documentation, and patient educationQualificationsLVN Candidates:Active Texas LVN license (required)Knowledge in patient assessments, medications, and care plan implementationMA Candidates:Completion of an accredited Medical Assistant program (preferred)Proficiency in vitals, documentation, and clinical support tasksFor All Candidates:Ability to coordinate with healthcare providers and ensure continuity of care Strong verbal and written communication skills Ability to educate patients and their families about health management and care plans Excellent organizational and time management skillsDesire to work from home
