Deliver compassionate, patient-centered, chronic care management to geriatric population in their homes with HarmonyCares, a leader in home-based primary care. Primary Care Physician Provide comprehensive primary care to seniors in their homes with a Medical Assistant partner. Manage a smaller patient panel (5-6 per day) for personalized care. Spend up to 45 minutes per visit to build strong patient relationships. Company-provided car for all home visits Work Monday-Friday, no weekends. Physician Leader Clinical oversight of Advance Practice Providers Oversee clinical operations, mentor providers, and ensure quality standards. Maintain a reduced patient panel while focusing on leadership duties. Monday-Friday schedule with no weekends.
