The territory will be Queens NY. RN Must have a Car and DL in good standing to be considered. In this role, the RN will coordinate, implement and continually evaluate the home care needs of your patients. You will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers. Assess the patient for the appropriate level of care and utilization of agency resources. Perform a comprehensive physical, psychosocial and safety assessment via examination, observation and motivational interviewing. Medication reconciliation Verify the initial plan of care with the patient"s physician; utilizing patient specific parameters inform the physician of any changes in the patients" status; update the plan of care to reflect changes made by the physician. Ensure the EMR accurately reflects: the current plan of care; the services provided; patient"s response to treatment; status of goal achievement. Accurately complete all OASIS data sets, within agency established guidelines, which reflect positive clinical outcomes and consistent utilization of industry established best practices.Interested in hearing more?
