This exciting role is integral to ensuring the highest level of care and service for our residents. The successful candidate will be responsible for coordinating and providing comprehensive service programs for residents in our residential healthcare facilities. This position offers the chance to make a real difference in people's lives, working within a supportive, diverse and inclusive environment.Responsibilities:As a Permanent Resident Service Coordinator, you will be responsible for:1. Developing, coordinating, and implementing comprehensive service programs that meet the physical, social, and psychological needs of residents.2. Advocating for residents' rights and needs within the community and the healthcare system.3. Collaborating with multidisciplinary teams to ensure residents receive optimal care.4. Providing ongoing assessment of residents' needs and preferences, adjusting services as necessary.5. Facilitating communication between residents, family members, and healthcare providers.6. Coordinating with external agencies to provide additional services as needed, such as social services or mental health services.7. Ensuring compliance with all federal, state, and local regulations regarding resident care.8. Promoting a positive, inclusive, and supportive environment for all residents.9. Training staff members on resident service protocols and best practices.10. Conducting regular meetings with residents and their families to discuss services and address any concerns or issues.Qualifications:To be considered for the Permanent Resident Service Coordinator position, candidates must possess the following:1. A Bachelor"s degree in Social Work2. Exceptional communication and interpersonal skills, with the ability to effectively communicate with residents, their families, and healthcare providers.3. Strong organizational and multitasking skills, with the ability to manage multiple priorities and deadlines.4. Proven experience in advocating for individuals' rights and needs within the healthcare system.5. Knowledge of federal, state, and local regulations regarding resident care.6.. Ability to work effectively with a diverse population and adapt services to meet individual needs.7. Proficiency in using computer software, including Microsoft Office Suite and healthcare databases.8. Demonstrated leadership skills, with the ability to train and guide staff members.19.
