A recognized health services organization on Long Island is currently seeking a dedicated professional to join their Health and Recovery Plan (HARP) team in Nassau County as their new Care Management Supervisor. In this role, the Care Management Supervisor will be responsible for the supervision of care management staff and the daily operations of the care management program. About the **Assignment * Temp-to-Perm* Monday to Friday 9am to 5pm Onsite at least 2 days a week Hempstead, Nassau, and Western SuffolkSupervise a team of six Care Managers, each managing a caseload of 35 clientsProvide guidance, feedback, and support to Care Managers in high-risk crisis situationsConduct fieldwork and client visits throughout Nassau CountyEnsure care managers are trained and equipped to handle their responsibilities effectively.Carry a small caseload of clients, as needed.Assist the Care managers with the development of effective clinical interventions and care plansProvide crisis intervention to clients in distressParticipate in quality assurance and quality improvement activitiesConduct staff meetings, trainings, and supervisionFacilitate bi-weekly Clinical Case Review with the Care ManagersPerform other duties as requested3 years of experience in Care Coordination or OMH Residential Services for AdultsBachelors Degree in Social Work, Psychology, Human Services, or a related fieldExperience in a Supervisory rolePost-graduate experience working with individuals or families who have a history of Mental Illness, Substance Use, Homelessness, or HIV/AIDSExcellent problem-solving, decision-making, and judgment skillsExcellent interpersonal and time management skillsAbility to work on an interdisciplinary teamDesired ***2+ years of experience working with the Mentally Ill populationMasters Degree in Social WorkNYS Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW)Bilingual (English and Creole or Spanish)
