Director of Care ManagementSalary ***** $110,000 - $135,000 annually (commensurate with experience and certification)Bonus ***** Quarterly bonuses based on hospital performance metricsFull-Time | Inpatient Rehabilitation SettingA leading 232-bed inpatient rehabilitation hospital is seeking a dynamic and experienced Director of Care Management (DCM) to lead a team focused on optimizing patient outcomes, discharge planning, and interdisciplinary collaboration. This role oversees a high-functioning care management department with an average daily census of 180 patients, including a 38-bed skilled nursing unit.Position ** The Director of Care Management will be responsible for the strategic and operational leadership of the care management team, which includes 27 direct ** case managers, case management assistants, and rehabilitation counselors. The DCM will not carry a personal caseload, allowing full focus on leadership, process improvement, regulatory compliance, and system-wide coordination of care.**Must be licensed as a Registered Nurse (RN), Occupational Therapist (OT), Physical Therapist (PT), Speech Therapist (ST), or Rehabilitation Counselor.Candidates from the MSW/Social Work discipline must have exceptionally strong leadership experience in healthcare case management and be Certified Case Managers (CCM).Certification as a Certified Case Manager (CCM) is required upon hire or must be obtained within the first 12 months of employment.Proven leadership experience managing teams within an inpatient or post-acute healthcare setting.Strong understanding of care transitions, utilization management, and discharge planning in a hospital environment.Key *Direct and manage the care management department to ensure high-quality, cost-effective, and patient-centered care coordination.Lead and mentor a team including case managers, CM assistants, and rehab counselors.Collaborate closely with hospital leadership and medical staff to meet clinical and financial benchmarks.Monitor regulatory compliance and continuously improve processes related to care transitions, length of stay, and discharge outcomes.Report directly to the Chief Executive Officer (CEO) of the hospital with a dotted line to the Regional Director of Care Management.Compensation & *Salary ** $110,000 - $135,000 annually, based on qualifications and certification.Quarterly bonuses based on achievement of financial and clinical performance targets.Comprehensive benefits package including medical, dental, vision, 401(k), and more.Professional development support, including assistance for obtaining CCM certification.If youre a strategic leader with a passion for advancing care coordination and outcomes in a rehabilitation-focused setting, we encourage you to apply.
