Social Worker (LMSW) - Internal Medicine

A healthcare network in New York City is looking to add a Licensed Master Social Worker (LMSW) to join their growing Inpatient Medicine Department. In this role, the Social Worker (LMSW) will be responsible for identifying psychosocial issues and concerns of patients and their families and providing appropriate clinical services to meet their needs.About the **Start * ASAPAssignment * 13-week contract (possible extension) Monday to Friday 9am to 5pm Acute Care Inpatient MedicineScreen charts for historical and clinical informationConduct psychosocial evaluations through interviews with patients and/or identified surrogatesFormulate appropriate social work treatment planAssist in plans for institutional or hospital discharge and other post institutional/hospital careProvide outreach services and make referrals to other governmental, public and/or community agenciesPrepare patients to accept services, provide direct social work services on high-risk cases, and may escort patients in connection with community activities and referrals, as neededPrepare reports and related paperwork; keep appropriate recordsPerform other duties, as needed***1+ year of Social Work experienceNYS Licensed Master Social Worker (LMSW)"‹Prior experience working as a LMSW in schools, child welfare agencies, criminal justice settings, shelters, other social service agenciesAbility to work within Microsoft Word, ExcelGood organization and writing skills as well as ability to work in a fast paced environment required.
