Job ** Home Health Physical Therapist Assistant (PTA)Reports * Rehabilitation Director, with supervisory input from a licensed Physical Therapist**** Patient Homes / Field-based - Lady Lake, FLJob PurposeTo assist in delivering rehabilitative services as directed by a licensed Physical Therapist. The PTA supports the implementation of individualized care plans aimed at improving patient function and quality of life in the home setting.Key ResponsibilitiesProvide therapeutic services under the supervision of a licensed Physical Therapist in accordance with established care plans and agency standards.Implement treatment plans, monitor patient progress, and report changes to the supervising PT and care team.Teach and counsel patients, families, and caregivers regarding therapeutic interventions and functional goals.Evaluate patient equipment needs and assist with implementation.Prepare and submit accurate and timely clinical documentation and progress notes.Participate in care coordination with other healthcare professionals, ensuring patient and caregiver involvement.Assist with discharge planning in accordance with care plan goals.Observe infection control and safety practices per organizational policies.Participate in in-service training and agency quality improvement initiatives.Perform other related duties as assigned.QualificationsGraduate of an accredited Physical Therapist Assistant program.Current PTA license in the State of Florida.Minimum of two (2) years of experience in a healthcare setting.Strong written and verbal communication skills.Valid driver's license and reliable transportation with active auto insurance.Physical RequirementsAbility to read, speak, and hear effectively to perform job duties.Physically able to stand, sit, bend, lift, kneel, and climb stairs.Must be able to travel to patient residences and work in home environments.Must be comfortable working with individuals who are ill, disabled, or emotionally distressed.Willingness to participate in ongoing professional education and development.
