A health services network in New York City is looking to add a Licensed Practical Nurse (LPN) to join their team at one of their foster homes in the Bronx as a Case Manager.About the **Start ** ASAPAssignment ***** Ongoing (Contract to Hire)** Monday to Friday** 9am to 5pmPatient ***** Pediatrics in Foster Care**Monitor each childs health and medical treatmentFollow up with the medical provider and the foster and/or biological familyProvide support, counseling and training to foster parents in health and medical needs of childrenCoordinate internal services with outside medical agenciesMake monthly home visits as well as specialty clinic appointmentsMaintain and update all required files and records1+ year of experience in an Acute, Sub-Acute, or Intensive Care role/settingGraduate of an accredited School of Practical NursingNYS Licensed Practical Nurse (LPN) licenseDesired ***Bachelors Degree in NursingBilingual (English and Spanish)Experience working with the Pediatric populationExperience with visiting Nurse Services
