You will play a critical role in coordinating care, supervising nursing staff, and implementing best practices to enhance patient well-being and reduce hospital readmissions.***** $135,000 - $140,000/year***** New York, NY***** Monday to Friday, 8:30 AM - 5:30 PMResponsibilities as the RN Clinical **Care Transition * Collaborate with hospital discharge planners, physicians, and care teams to develop and implement safe, patient-centered transition plans.Clinical Assessments & Home * Conduct home visits to assess patient health, living conditions, and support systems, ensuring continuity of care.Nursing Care & Health * Provide skilled nursing services, including medication management, wound care, vital sign monitoring, and chronic disease education.Supervision & Staff * Lead and mentor Nurses, PCAs, and HHAs, ensuring competency, compliance, and adherence to best practices.Regulatory Compliance & Quality * Maintain accurate documentation, ensure compliance with PACE and LHCSA regulations, and drive quality improvements.Patient & Family * Educate patients and families on care plans, medication adherence, and lifestyle modifications to promote effective self-management.Qualifications for the RN Clinical ** Degree from an NLN-accredited nursing school required (BSN preferred). 3+ years of clinical nursing experience, including 1-2 years in home care, care transitions, case management, or PACE programs (long-term care or managed care highly preferred).
