Home Care Planner

MJHS is a large not-for-profit health system in the Greater New York area. We also offer Elderplan/HomeFirst: health plans for Medicare and dual-eligible individuals. You will manage and facilitate the transition of patients onto the home care program in accordance with applicable law, regulations and agency policies and procedures.Meets the established Monthly Admission Targeted GoalsProvides flexibility within the role of effectively supporting the agency’s overall business objectives and goalsEstablishes a plan of care (POC) in collaboration with patients, physicians and members of interdisciplinary health care team and arranges for all needed services under the direct supervision of an HCP Manager and/or Director of Business DevelopmentWorks in conjunction and maintains consistent communication with discharge planning staff to identify problems that may impede the transition of the patient returning home and assists with resolution as needed; under the direct supervision of an HCP Manager and/or Director of Business DevelopmentCommunicates agency admission criteria, reimbursement criteria and policies to patient/caregiver, members of the interdisciplinary health care team and other personnel as indicated; under the direct supervision of a HCP Manager and/or Director of Business DevelopmentConducts an evaluation of referral needs to determine potential alignment with additional MJHS products and services ( Elderplan, HomeFirst, Hospice, Palliative Care); under the direct supervision of an HCP Manager and/or Director of Business DevelopmentTo identify and assess referrals received for the various MJHC clinical service programs (e.g. Heart Failure, COPD, Joint Replacement, Diabetes Management, Community Based Palliative Care) as deemed applicable; under the direct supervision of an HCP Manager and/or Director of Business DevelopmentTo identify and assess that the Home Health Face-to-Face documentation as mandated by the Centers for Medicare and Medicaid Services (CMS) is in all Medicare patient’s medical record under the direct supervision of an HCP Manager and/or Director of Business DevelopmentManages the referral process, ensuring timely and accurate coordination between departments to optimize patient care and streamline operations; under the direct supervision of an HCP Manager and/or Director of Business DevelopmentAssociates in Nursing; Baccalaureate Degree preferred.Minimum of one year of acute, Medical-Surgical nursing experience, preferred.Home Care experience preferredSpecialized knowledge in Microsoft Office, (Word, Excel, Outlook, etc.)By applying, you consent to your information being transmitted by StaffAttract to the Employer, as data controller, through the Employer’s data processor SonicJobs.See MJHS Privacy Policy at and SonicJobs Privacy Policy at and Terms of Use at
