Home Health RN

The Home Health Registered Nurse (RN) is responsible for delivering high-quality, patient-centered care in the home setting. This role involves performing comprehensive assessments, developing and managing individualized care plans, and coordinating with an interdisciplinary team to ensure optimal patient outcomes. The RN will utilize OASIS documentation, provide education to patients and families, and monitor patient progress while working independently in the field. Strong clinical judgment, time management, and communication skills are essential for success in this role. -Provide and coordinate skilled nursing care in patients’ homes in accordance with physician orders and agency guidelines -Perform comprehensive patient assessments (OASIS) and develop individualized plans of care -Collaborate with physicians, therapists, and interdisciplinary team members to ensure continuity of care -Complete accurate and timely documentation in Homecare Homebase -Supervise and delegate tasks to LPNs and home health aides as appropriate -Educate patients and families on disease management, medications, and care plans -Ensure patients meet home health eligibility and medical necessity requirements
