The At-Risk Mentor provides case management and support to students within their portfolio, building high quality, supportive and challenging relationships, and connecting with other adults in their life to help support holistic strategies for development and success. í¢€– Responsibilities Include: Utilizing strategies such as home visits, collaboration with classroom teachers and clinical professionals, and exploration of afterschool and community-based resources to develop a comprehensive understanding of each studentí¢€™s academic, social, and home environment. Building and sustaining strong, trust-based relationships with multiple stakeholdersí¢€”students, families, educators, and service providersí¢€”while operating independently and maintaining a clear focus on student achievement. Reviewing and interpreting student-level data to assess the impact of interventions, monitor student progress, and inform ongoing academic and behavioral support strategies. Connecting students to tailored supports and interventions, conducting consistent follow-up, and strengthening engagement with families, teachers, and external stakeholders to ensure cohesive and effective care. Identifying students who are disengaged or chronically absent through home visits, relationship-building within the Ombudsman setting, and use of referral data, followed by designing and implementing individualized intervention plans. Leading comprehensive case management for students re-entering Ombudsman after suspensions, home stays, or extended absences, ensuring smooth transitions and aligned supports for academic and behavioral reintegration. Participating actively in the Clinical Care Team, contributing insights and collaborating on holistic support plans that address students' emotional, behavioral, and academic needs. Facilitating regular meetings with security personnel and advocate associates to discuss student-specific concerns, evaluate intervention effectiveness, and promote a safe, supportive learning environment. Utilizing data systems to monitor student performance, track progress toward goals, and recommend additional interventions grounded in evidence-based practices. Performing other duties as assigned in alignment with the evolving needs of students and the broader school community. í¢€– Qualifications Required: High school diploma or equivalent with passage of basic skills test. Hold currently or have the ability to obtain a valid IL state driver's license. Prior experience and/or knowledge in alternative educational services for at-risk students with diverse needs at various levels. Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals and next steps. Highly skilled in building relationships with students, parents, teachers and/or community partners. History of success building credibility, making quick decisions, delivering fast results and demonstrating resilience and persistence to get things done. Proven ability to effectively prioritize tasks and meet deadlines while maintaining organization and attention to detail. Well-versed in mentoring, coaching, administrative support, and/or technical support functions. Advanced oral and written communication, organization, conflict resolution, multitasking, problem solving, and decision-making ability. Knowledgeable in the use of relevant technology including experience with computers, Microsoft Office Suite, database entry and basic office equipment.
