We seek physicians who are committed to delivering high-quality, community-focused care while achieving positive health outcomes for their patientsKey ResponsibilitiesAs a Primary Care Physician (PCP) and leader of the care team, you will provide comprehensive primary healthcare to a dedicated patient panel while ensuring exceptional, value-based care to the community.Deliver patient-centered, comprehensive primary care services aligned with a value-based care modelActively participate in quality improvement programs and contribute to practice performance metricsMaintain compliance with privacy, regulatory, and quality standards, ensuring adherence to HealthyU Family Medicine policies, procedures, and protocolsImplement and support innovative healthcare technologies, services, and programs to enhance community health and wellnessCollaborate with clinical and administrative teams to ensure high-quality patient carePerform other duties as assignedRequirements Completion of an accredited Medical Program in Family Medicine or Internal MedicineValid state-issued medical license (must be active by the first date of employment)Current BLS (Basic Life Support) certificationActive DEA RegistrationAbility to provide two professional referencesTwo or more years of clinical experience in primary careExperience providing longitudinal patient care (continuity of care)Familiarity with managed healthcare systems, including quality assurance, quality improvement, risk management, population health, and disease management programsStrong ability to work collaboratively within a multidisciplinary teamExcellent communication skills with both patients and colleaguesSelf-motivated and results-driven approach to patient care
