This role offers a dynamic opportunity to lead within a large, fast-paced healthcare environment known for excellence in emergency care, cardiology, and surgical services.What You’ll Do:Provide daily leadership to a team of 15–18 Case Managers and Case Management AssistantsDrive patient throughput and proactively manage discharge planning processesCollaborate across interdisciplinary teams including physicians, families, and insurance providersEnsure compliance with all regulatory and organizational policiesEscalate and resolve delays in care progression and dischargeEducate and hold team accountable to care coordination standardsParticipate in on-call rotation for urgent coordination and service recovery needsWhat You’ll Bring:California RN license (pending license accepted)BSN required, MSN preferred3–5+ years of case management leadership or nursing management in an acute hospital settingExpertise in InterQual, M&R, and payer-specific discharge criteriaStrong understanding of Medicare, Medi-Cal, managed care, and private insuranceAbility to lead difficult discharge planning conversations and resolve care barriersReady to “hit the ground running?? with minimal trainingCompensation & Details:?
