With a clinical team of leading dialysis specialists, Concerto understands the unique needs of nursing home operators, providing effective dialysis care to skilled nursing facility (SNF) residents with end-stage renal disease (ESRD). The innovative model of Concerto overcomes challenges, resulting in improved processes and healthier outcomes for SNF dialysis patients.Summary/Objective Concerto is seeking an Executive Clinical Leader to join its team! This individual will serve as the senior clinical leader of the company, responsible for ensuring the highest standards of patient care, clinical innovation, regulatory compliance, and medical leadership across all dialysis services. This role will play a critical role in shaping the company’s care model, driving clinical performance metrics, and advancing technology-enabled care pathways.An ideal candidate is a strategic leader who can take ownership and initiative to improve processes as necessary. This candidate must be a strong team player and manager, who can lead a team and attract and retain talent as necessary. The position reports to the Chief Operating Officer. Essential FunctionsClinical Strategy & LeadershipDevelop and lead the overall clinical strategy in alignment with the company’s mission and goals.Champion innovation in dialysis care delivery, including Hemodialysis, digital health integration, and patient engagement models.Provide executive oversight to all clinical operations, including quality, safety, and patient outcomes.Care Model InnovationPartner with technology and product teams to design and implement tech-enabled clinical pathways.Lead initiatives to personalize dialysis care, including individualized treatment plans and predictive analytics for patient risk stratification.Quality, Compliance & SafetyEnsure compliance with all federal, state, and local regulations, including CMS, ESRD, and HIPAA requirements. Oversee clinical quality improvement programs and patient safety initiatives.Guide the development and execution of clinical performance dashboards and KPIs.Team Building & Clinical CultureRecruit, develop, and mentor a high-performing clinical team.Foster a culture of clinical excellence, accountability, and continuous improvement.External Engagement & Thought LeadershipRepresent the company in external forums, including partnerships with healthcare systems, payors, and policy groups.Stay abreast of emerging trends in nephrology, telemedicine, and value-based care models.PHYSICAL DEMANDS AND WORK ENVIRONMENT:·       Must be able to do light lifting of at least 20 lbs. ·       Requires mental, visual, and auditory attention to patients, customers, and staff. ·       Work environment is characteristic of a health care facility with air temperature control and moderate noise levels. ·       May be exposed to infectious and contagious disease/materials. ·       Requires travel between facilities.QUALIFICATIONS:·      Minimum of 10 years of clinical leadership experience, preferably in Dialysis, Kidney Care, or Chronic Disease Management·       RN licensure·       Understanding of CMS regulations·       Proven track record of leading clinical teams and driving improvements in care quality, safety and operational efficiency·       Experience with ESRD and HD, along with digital health is strongly preferred·       Deep knowledge of healthcare regulations, dialysis reimbursement models and value-based care frameworks·       CPR certificationWhy work at Concerto?·       Medical, dental, vision, short and long-term disability benefits·       401k matching program·       Unlimited PTO·       Paid holidays·       Discounts through Nectar·       Employer Paid Employee Assistance Program Other DutiesPlease note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
