The role focuses on effective patient assessments, needs evaluations, discharge planning, and DME orders to ensure continuity of care and efficient transitions.Key Responsibilities:Conduct comprehensive needs assessments and discharge planning for medical inpatientsCoordinate care and services in collaboration with the interdisciplinary teamArrange for post-discharge services, including ordering durable medical equipment (DME)Float to different MedSurg units within the patient tower as neededMaintain compliance with CMS and HIPAA standardsEnsure accurate and timely documentation of case management activitiesRequirements:Licensure:Active Maryland licenseCertifications:BLS (Basic Life Support) – RequiredFlu vaccine – Required with no exemptionsExperience:Minimum 2 years as a Case Manager in Outpatient clinic and acute hospital settingsExperience in care coordination, discharge planning, and DME orderingMedical inpatient experience requiredAdditional Details:No on-call requirementTraveler may pick up weekend shifts if desired and available*Offered pay rate will be based on education, experience, and healthcare credentials.Interested?
