The Home Health Clinical Educator, RN will partner with our field staff and leaders to provide the best support to the patients that we serve. This position will be in our Grand Rapids, MI office location.Home Health Clinical Educator, RN Job Responsibilities: Trains in assessment, planning, intervention, and evaluation techniquesCommunicates with Staff Development Manager & Specialists related to orientee’s completion of the field education guideAssists Staff Development Specialist in the development of orientee’s schedule to include scheduled visits with other departments in agencyEducates orientee on how to develop and revise the plan of care in response to changes in the individual's status or response to care, and communicate and update physician and other disciplinesReviews charting expectation and computer documentation with new providersInstructs orientee in case management practices; organize daily assignments, setting priorities based on individual and overall needs of the clientsCoordinates assistance from preceptors as necessaryMentors current employees with any identified learning need in day-to-day operations.Qualification Requirements: RN License in the state in which you workPrevious Home Health Experience PreferredPrevious Education Experience PreferredAt least 2 years of nursing experienceAbout Graham Healthcare Group: As an innovator in an evolving healthcare world, Graham Healthcare Group has been designing business and technology solutions to drive better care, outcomes, and productivity within its own home health and hospice companies for over 20 years.
