Job Title: Lead Primary Care Physician (Temp-to-Perm) Location: Maricopa County, AZ (with potential visits to Pinal County)Job Type: Full-Time, Temp-to-Hire (6-month contract with potential for permanent placement) Work Setting: Hybrid – Majority remote with some in-field responsibilities Compensation: $105.00–$120.00 per hour Schedule: Monday–Friday, 8:00 AM to 5:00 PM (some weekend secondary call coverage required)Position start date: ASAPAbout the RoleWe are seeking a Lead Primary Care Physician to join our care team and provide clinical oversight and support to nurse practitioners while also delivering hands-on care to patients with complex medical needs. This hybrid role combines virtual collaboration with occasional in-person visits, primarily in Maricopa County with potential travel to Pinal County.The ideal candidate brings at least three years of primary care experience, excels in managing chronic conditions and polypharmacy, and demonstrates leadership and adaptability in a value-based care environment.ResponsibilitiesClinical Leadership and OversightProvide direct supervision, coaching, and mentorship to advanced practice providers and physicians.Ensure high-quality, value-based care and accurate documentation.Oversee clinical education and continuous improvement initiatives.Deliver care to medically complex patients as needed (typically 0.5–2.5 days/week).Care Coordination and CommunicationCollaborate with specialty care teams, local health plans, and community resources.Support data-driven multidisciplinary care planning.Participate in local outreach and development initiatives.Operations and Quality ManagementPartner with operational leadership to optimize care delivery and team performance.Contribute to budgeting, protocol development, and achievement of quality benchmarks (Team Experience, Patient Satisfaction, Better Outcomes, Lower Costs).Uphold compliance and promote a culture of excellence and integrity.Patient CareDiagnose, treat, and manage acute and chronic illnesses in home- or community-based settings.Maintain up-to-date medical knowledge and evidence-based practices.Ensure privacy and confidentiality in all patient interactions and records.QualificationsRequired:Graduate of an accredited medical schoolBoard Certified in Internal Medicine, Family Medicine, or Medicine/Pediatrics (Board certification not required in Puerto Rico market)Minimum 3 years of primary care experience managing chronic conditions and multiple medicationsExperience in Skilled Nursing Facility (SNF) settings is a plusActive and unrestricted medical license in Arizona (or ability to obtain within 90 days)Valid DEA certification (state and federal)Current AHA or ARC BLS certificationStrong EMR documentation and computer proficiencyMust pass background check and drug screeningValid driver's license and proof of insurance for travel within service areasPreferred:Hospitalist experience or exposure to public health, healthcare administration, or business administrationExperience working with underserved populations, behavioral health, and substance use disordersKey CompetenciesExceptional leadership, coaching, and communication skillsStrong understanding of population health and value-based careAnalytical and solutions-focused approachFlexibility in adapting to evolving healthcare needsPassion for compassionate, patient-centered carePhysical & Working RequirementsAbility to lift up to 20 lbs., bend, kneel, and remain stationary for periods of timeFrequent driving to patient homes and community sitesOccasional weekend phone-based call coverageMust be able to effectively communicate, document, and make clinical decisions under pressure
