Licensed Master Social Worker

We are seeking a dedicated and passionate Licensed Social Worker (LMSW - $66+/hour!) to join our team for hospital contract positions around NYC. These are full-time, M-F/9a-5p roles with contracts available in Manhattan, Brooklyn, Bronx, and Queens.(Hospital) LMSW Social Worker Duties Include:Reviewing admissions information to identify patient needs, such as special diets that align with personal or religious beliefsServing as an advocate for patients and advising members of the medical care team about patients’ needsConducting discharge counseling to ensure patients have the proper support, accommodations, supplies and equipment when they return homeReferring patients to government agencies and nonprofit organizations that can assist with homelessness, food insecurity, unemployment or lack of transportationCoordinating care between medical providers for complex cases(Hospital) LMSW Social Worker Requirements:Master's Degree in Social WorkLMSW required1-2 years prior experience as a Social Worker in a hospital/acute care setting
