This is an exceptional hybrid opportunity that is mainly within a clinical/outpatient setting but would also involve working in the field approximately 25% of the time. Key Responsibilities:Provides clinical physical therapy evaluation and treatment plans for program participants 55 and older, on an acute restorative or maintenance level as neededResponsible for integrating physical therapy treatment plans into the Plan of Care (POC) as directed by the physician orders.Collaborates with nursing home and assisted living staff regarding the rehabilitation needs of patients Key Requirements:Bachelor’s Degree in Physical Therapy required, Masters in Physical Therapy preferredBilingual preferredCompletion of a physical therapy program accredited by the American Physical Therapy Association (APTA), the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association or other equivalent organization approved by CMSCurrent, active Physical Therapist license within state of practiceA minimum of one (1) year experience working with the elderly Compensation and Schedule:Pay rate starts at $100K but can be considerably higher based on years of experience Comprehensive benefits plan Monday-Friday, 8AM - 4:30PMPaid holidays and NO weekends
